There is a known bug with patch for a very similar problem. See
http://www.squid-cache.org/Versions/v2/2.5/bugs/
First make sure you have this patch applied. If the problem persists
please register a bugzilla entry, preferably including a stacktrace (see
the Squid FAQ on how to get stack traces).
