I had the same problem.
After the clients recognized the proxy server the rights at the icons
directory are not good.
I had played with the right before and forgotten to restore them to default.
You have to set the /usr/local/squid/share/icons directory to chmod 755 and
the files within the directory to chmod 644.
The owner of the directory and files is root.
Take a look to cache.log too, errors of loading the icon files are reported
here.
