This line indicates you have a client sending malformed requests to
Squid. To confirm/deny this, enable log_mime_hdrs and then watch the
Proxy-Authorization header for odd values..
Note: DO NOT post access.log lines in public with log_mime_hdrs
enabled as this will contain user private details such as login names
and passwords.
