The error means squid read in a cache file and, it turned out, the URL
stored there is not what squid thought was stored there. It could be
the swap.state was corrupted, or the file is pointing to the wrong disk
blocks (fs corruption).

To start, shutdown squid, delete the swap.state, and start squid, so it
can rebuild the state by reading the cache files. If this continues to
happen after that, either the filesystem or the disk is toast.
