What is the setting of

   /proc/sys/net/ipv4/tcp_max_syn_backlog
   /proc/sys/net/ipv4/tcp_abort_on_overflow
   /proc/sys/net/ipv4/tcp_syncookies

I assume you have already verified that it was not caused by a Squid
restart, and that there is no IP conflicts or outer routing problems
causing traffic to sometimes hit another server etnirely.
