Depends on very many factors, but in the current design CPU efficiency
degrades quite noticeable when going above some thousands of concurrent
connections.

There is hope this will be considerably improvved in the upcoming
Squid-3.0 release with the help of epoll/kpoll.

One does not exclude the other..

ipvs for distributing the clients among the caches in a easy to administer
manner.

peering to improve cache hit ratio by sharing cache content.
