Last time there was a report like this it was isolated to a anti-virus
scanner which did not implement persistent connections properly, keeping
the connection open to the previous web site even if the next request is
for a completely different web site. This was with another brand of
anti-virus scanner however.

If this is your problem it may help to set "server_persistent_connections
off" in squid.conf.
