Well.. the old was quite likely corrupted by both including false
information and not including all information.. this is not unlikely in
case of power failure or other uncontrolled system restart.

When you rebuilt swap.state Squid rescanned the cache directory, adding
whatever it found in there.

It should also be noted that swap.state is a kind of transaction log and
grows until "squid -k rotate" where the transaction log is reset.
