[squid-users] squid "via" "x-forwarded-for" headers missing

I am using squid2.5 stable5 with all current patches. The via and x-forwarded-for headers are missing from the squid server. Squid should put in (or append to) a via or x-forwarded-for header for itself right? I am not using the "header_access" feature. The default is to allow all headers. Any suggestions on why this might be happening?
