 Squid version ?
 OS/platform/version ?

 Make sure that , for instance, squid is not suffering from slow DNS lookups
 and or problems related to DNS resolving.
 Check cache.log for this and cache mgr -> Internal DNS statistcs.

 Check cache.log for possible other errors too, if any.
