The system log does log an error: Squid Parent: child process 3289
exited due to signal 6? What would cause this?

There is not much difference.. If the logs works in one mode they must
work in the other.

Are you positively sure there is nothing in either cache.log or your
system messages file?

Do you get log information in cache.log when you use squid -N?
