Sorry, I had:

external_acl_type propel_header_auth %{Hdr:member} /etc
/squid/ident.sh x-pun:user

acl propel_header_auth ident REQUIRED

http_access allow propel_header_auth

as well and still not working -- I think. I am still getting:

SQUID ERROR: ERR_ACCESS_DENIED 0 ACCESSING: http://...

Is there some way to tell if it is working?
