Usually the cause to this kind of problem is incorrect time on either the
proxy or the web server. Use of NTP to keep the clocks running proper is
strongly recommended.

If this doesn't help then access.log with "log_mime_hdrs on" provides the
information needed to analyze this kind of problems. The cachability
check engine is also quite helpful in diagnosing this kind of problems.
