Cured by the following two patches:

http://www.squid-cache.org/Versions/v2/2.5/bugs/#squid-2.5.STABLE10-transparent
http://www.squid-cache.org/Versions/v2/2.5/bugs/#squid-2.5.STABLE10-NetBSD_IPFilter

or wait for 2.5.STABLE11.
