[squid-users] Failure Ratio?

Hi all,
         I have a strange problem with the Failure Ratio messages --

2006/08/30 02:41:09| Failure Ratio at 1.37
2006/08/30 02:41:09| Going into hit-only-mode for 5 minutes...
2006/08/30 02:46:14| Failure Ratio at 1.46
2006/08/30 02:46:14| Going into hit-only-mode for 5 minutes...
2006/08/30 02:51:20| Failure Ratio at 1.52

This has occurred on a cache of ours and continued to throughout the night.
However, upon restarting it everything began to work fine again.

If there was some real failure, would it not have automatically restarted to
fetch requests? Given that after manually restarting it the cache worked
fine we can assume the link was not down at that stage? It also has no
peers..
