Has wrote Henrik, you must use the command "htpasswd -m"
But the version of htpasswd provided into NCSAsupport.zip doesn't
support -m option.

You must add libcrypt to your MinGW as in the my previous message and
rebuild Squid or you can search for an updated htpasswd.exe, try the
apache web site.
