Why have you build accelerator mode Squid's with ICMP support?

This means that the icmp pinger hasn't been installed properly.

TIMEOUT_ means ICP (not ICMP) timed out.

FIRST_UP_PARENT is the peer select method Squid selected after the ICP
timed out.
