Your statistics is a bit off due to the NTLM auth messing with things..
You have to subtract TCP_DENIED from the results.

Corrected statistics:

HTTP: 3140796-2678413=462383 Requests, 83031 Hits ( 18%)

But even considering that you use NTLM you have an abnormal amount of
TCP_DENIED... Have you perhaps disabled client-side persistent
connections making NTLM show it's worst face?
