After looking at the Squid logs a little closer I think I have a
better idea about where the problem is occurring. Here is the
important part of my access.log:

The first two requests to owssvr.dll return an HTTP 401 unauthorized,
and you can see how it sits there for 60 seconds and then makes a
totally different request to author.dll. This is what I was talking
about when I said the save/save as dialog hangs and looks different
when using SharePoint Services through Squid rather than going direct.
It looks like Squid isn't passing the users credentials when
performing HEAD requests. Or perhaps there is another problem that is
preventing this from working?
