Two possibilities I can think up:
1) Squid is going to a different server than your
browser-bypassing-squid request.
2) The file was missing on the original request through Squid, and
subsequent requests are getting the cached response (note the
TCP_NEGATIVE_HIT). Check your negative_ttl setting (it defaults to 5
minutes).
