I have not changed the refresh_pattern, I didn't feel yet I have to play
with it.

should not be a problem, but note that cache_mem applies only for object
fetched from network, so you may decrease it, if you have big disk cache, to
lower memory usage and allow OS to cache files from dis cache efficiently.

they depend on youe cache size and cache_mem. I use 32MB/128KB maximum file
sizes with 48GB/128MB of cache sizes, however I only guessed these values...

(long time ago I was searching through access.log to see how big files got
repeatedly fetched in a short time, I probably should do this again, and I
think I'll change maximum_object_size to something higher)

with 2GB of memory you can use ~100GB cache_dir without problems. see

squid memory usage.
