Hi!

The ICAP stuff is an addon patch to Squid-2.6.17 (as it doesn't support
ICAP natively) and thus we don't support it.

You'll have to contact the ICAP patch author or look at trying out Squid-3.0.
(Anyone know who looks after the Squid-2.6 + ICAP patch these days?)
