[squid-users] Cannot Replicate Error

Here is an DENIED error that I keep getting from random IPs

TCP_DENIED/400 1409 OPTIONS * - NONE/- text/html

I cannot reproduce the above using

- telnet proxyserver 80
.... GET * HTTP/1.0
.... OPTIONS * NONE/

- setting web browser to proxyserver on port 80 and accessing *

Note: Squid is setup as a reverse proxy->webcluster

Does anyone know how to reproduce the error, and is this a know/potential
security risk?
