Forwarding loops is logged very aggressively in cache.log as such, and
don't result in an error to the user. All Squid does on a forwarding
loop is to try to go direct to the origin.

The error message seen here was "Unable to forward" which means that
never_direct is in effect (on by default on accelerated requests), and
that it did not find a parent where to forward the request.
