It seems to be only 2 files: a perl script (squid-prefetch) and a config file(squid-prefetch)...
Just put them somewhere (like /usr/local) and make/copy an init.d script.
Forget about my urlrewrite suggestion, reading the acces log is indeed a lot easier...
