Check that you have "negative_ttl 0 seconds" configured. 2.x has a wrong
default of some minutes.

If the above setting is correctly at 0 seconds already, check the web
server provided headers.
4xx messages CAN be cached in a lot of cases, but it should be up to the
origin site to specify correctly when.
