IPv6 is irrelevant. At its core this is a TCP handshake issue. They
happen in IPv4 as well under the same PMTU, ECN, and Window Scaling
conditions.
The important part is the bits you left off the report so far ... where
Squid is being asked to connect to and what host/IP it is opening a TCP
connection to.
