It most likely occurs because of a bug in Squid code.

Please file a bug report with bugzilla, posting a gdb backtrace from the
core file created when a worker dies. If there is no core file or you
need more information on how to collect a back trace, please visit:

  http://wiki.squid-cache.org/SquidFaq/BugReporting

Please note that without a backtrace, your report will most likely not
contain the information necessary to triage and fix the bug.
