Comparing your info with my squid logs, the HEAD-req is missing in my
squid-log. So I will first do a second test, to make shure. And try to
disable cache in my squid.

What I notice in your fiddler, there is another squid3.1.12 in-between. When
you directly access
the video, without your squid, this squid3.1.12 also in the path ?
And, when you directly access the video, is the fiddler-trace the same,
incl. HEAD ?
