Yes for Squid-3.3 or later.

First you deny the existing header being sent (because HTTP only permits
one User-Agent header):
http://www.squid-cache.org/Doc/config/request_header_access/

Then you add the new one:
http://www.squid-cache.org/Doc/config/request_header_add/
