did you use external auth helpers?
we have (still) the problem that with squid 3.4.x squid will go up to 99% cpu usage.
when we deactivate external auth helpers squid will stay around 20%.
we have to switch back to squid 3.2.11 which works without problems.

today we did a test with the latest release 3.4.6.

delay_pools are not used at all (delay_access 1 deny all), i'll try to
remove them completely,
but as 3.3 works fine i doubt that'll help.
