hosts-file parsing

Squid can now read your hosts file at startup/reconfigure
Useful for overriding the DNS for accelerator setups
Defaults to /etc/hosts under UNIX
Defaults to %windir%\hosts under Windows 9x/ME
Defaults to %SystemRoot%\hosts under Windows NT/2000/XP
hosts_file /etc/hosts
