/etc/hosts parsing capabilities added to squid-sourceforge

available with the cvs tag etc_hosts from sourceforge.

It plays some modest tricks with the IP and FQDN
caches to allow a Squid using internal-dns to exploit
/etc/hosts.

Currently unconfigurable, it works for me. Please test
and if deemed appropriate merge.
