Seems your cache directory is only partially created, or that you deleted the
cache while Squid was running..

Hmm... how much old crap did you save in that cache.log???

Internal error. For some unknown reason the store client is out of synch with
itself.

Same error again.

Is this Squid version patched in any manner, or is it a plain
Squid-2.5.DEVEL/CVS version?

What were you doing at the time of crash?

Also, it would help a lot if you could get a stack trace of the crash.
