authenticate_ip_ttl confusion

I'm trying to understand authenticate_ip_ttl. Its description in
squid.conf leads me to believe that simply setting authenticate_ip_ttl
to non-zero makes Squid deny some requests.

However, reading the source code it seems that requests are
only denied if a max_user_ip ACL is used (and matched).
