New NTLM code - First feedback

Hi,

My first feedback after 3 days of testing of the new NTLM code merged in
the nt-2_5 branch using native NEGOTIATE packet support is very positive:

- no more authentication pop-ups at all
- the authentication process seems to be more fast
- the helper's usage seems lower

Tested configuration:

Squid running on Windows 2000 server, native Windows NTLM authenticators
with NTLM NEGOTIATE packet support enabled, 3 concurrent users
