If you open the document, and presses Reload in Mosaic or Shift+Reload
in Netscape, Squid completely reloads it from the source. Another option
is to slightly modify the actual document (touch it), and then press reload
in Netscape.

Try adding the machines own address as well, or use localhost as
the host in cachemgr.
