You need to look in cachemgr.c for the lines of the form :

printf("<BR><STRONG>Cache Host:</STRONG><INPUT NAME=\"host\" ");

to

printf("<BR><STRONG>Cache Host:</STRONG><INPUT NAME=\"host\" VALUE=\"phost\" ");

where "phost" is the name of your proxy host, etc...
