What does these log-fields mean?

I have tried to find this in the documentation, no luck or just plain blind.
We are having problems with java-classes that are cached when they shouldn't,
that's the reason I need this information.

We have Squid 1.1.3.

Btw, I have:
refresh_pattern/i \.class$ 0 0% 0
refresh_pattern . 0 20% 4320
Wouldn't this mean that files with names ending in .class are never cached?

The first four fields in cache/log:

000099ae 32ce9aa6 ffffffff 32b9d76a
0000199c 32ad5fa7 fffffffe 32ad5db8

I know that the first field is the filename. What is the algoritm for
getting to this file in the cache-hierarchy based on the name?

In store.log:
