Check your squid.conf ACL entries. Cachemgr.cgi is by default only
allowed from localhost (== 127.0.0.1).
If you are running the apache CGI server on the same machine, then use
localhost as your server name instead of your internet name (which is
not assigned 127.0.0.1).
This is probably in the FAQ somewhere...
