What's wrong with my Squid?

I am seeing poor squid performance at times and very high memory usage.
Should the Pool for disk I/O be so high? The cache is set to grow to 5000
MB ... currently there is only 600 MB cached. OS is Linux 2.0.33 patched
to allow 3000 file descriptors. The system has 64 MB of RAM and 64 MB of
swap. Page faults are very high, what could be wrong? I have allocated
30 MB of RAM in squid.conf, I do plan to add RAM to the system but didn't
think I'd have to until it grows closer to the 5000 MB limit I have set.

Cache information for squid:
Squid Object Cache: Version 1.NOVM.20
        Storage Swap size: 606 MB
        Maximum Swap size: 5000 MB
        Number of TCP connections: 166797
Page faults with physical i/o: 287434
