This beats me, but based on these log messages it looks like you have
enabled USE_BIN_TREE or USE_SPLAY_TREE. Try recompiling your Squid
without either of these (you don't need them unless you are using large
filtering lists, and the *_TREE code is much less tested than the normal
code).
