mistakes in configuration for MRTG in FAQ

Just a few mistakes.. there is no entry named "cacheHttpAllSvcTimeFiveMin"
as far as I can tell. I assume "cacheHttpMedianSvcFiveMin" was what was
wanted.

Also, the header reads "PageTop[httpsvc]: <H1>Cache Statistics: HTTP Median
Service Time (5 min)<H1>" when it should be "PageTop[httpsvc]: <H1>Cache
Statistics: HTTP Median Service Time (5 min)</H1" (it's missing the / in the
<H1> at the end).
