Squid Segment Violation (and subsequent death)

On our Linux 2.0.35 (Redhat 5.1) PentiumII 128MB RAM machine, squid, at
times of high traffic, dies unexpectedly.

The message in cache.log is:

...
FATAL: Received Segment Violation...dying.
1998/07/30 14:00:39| storeWriteCleanLog: Starting...

After which point it writes out it's cache stuff and dies. What is
causing this? We are using squid 1.1.21 (rebuilt from the SRPM)

Thanks
