How do I know if my hard disk is swapping

hello,

        How can I determine when my hard disk begins to swap.
    What are the steps to prevent my system from swapping.

        I have been running 1.1.22 version on P-II 300 with
    128 Ram. My cache_mem is 32 and my cache_swap
    is 4 gig. I get around 15,000 to 20,000 hits a day.
