It's very likely (in fact, if I read the docs correctly, certain) that the
object you're requesting is older than the minimum time in the
refresh_pattern.

Squid verifies that objects have not changed sinced the cached version was
obtained _every_ time the object is requested. That's what's triggering
the dialup.

[AFAIK. But it's what I found emperically with diald + squid]
