As I compiled a list of proxies I knew of, I noticed few filtering proxies.
 Here are a couple:

Junkbusters provide a proxy that can block advertising, cookies, etc.:

Muffin, written for Java 1.1+, has a filter to block Java and JavaScript:
http://muffin.doit.org/doc/filter/Decaf.html
and another relevant filter:
http://muffin.doit.org/doc/filter/NoCode.html
These could probably be rewritten to block ActiveX.

Both of these are under GPL, and Junkbusters' would seem to be scalable;
rewriting Squid looks redundant.
