Latest stable release is 2.2.STABLE2.

ICP queries are not forwarded to parents (or siblings). Only if a HTTP
request is received then Squid generates a ICP request to it's parents.

It says NONE in the hierarchy field, since ICP requests is never
forwarded.

Try repeating the tests using the client program delivered with Squid.
Most people get confused at first when trying to verify things using a
browser. Also note that the ICP peerings in Squid does not work very
well unless Squid is put under some load, so testing with single
requests are likely to fail.
