Forwarding loop occurs when request goes through a proxy more than once. It
is discussed in FAQs.

Hello,
    I am running Squid 2.2stable2 on Solaris 2.7 x86 edition. It has
operated very well for about 2 days, but after that it slowed down suddenly
almost to a halt... The attached warning appeared a lot in cache.log Then
the following appeared: "1999/05/17 13:43:50| WARNING! Your cache is
running
out of filedescriptors"
I am using it in transparent mode using an Alteon AceDirector2.
I want to know what is the meaning of "Forwarding loop detected" ? Is it a
routing problem or what??
