help for the squid

Hi,

setuped squid Version 2.1.PATCH2 and it working fine for few months. Now,
I have problem with:
FATAL: Write failure -- check your disk space and cache.log
Squid Cache (Version 2.1.PATCH2): Terminated abnormally.
CPU Usage: 4.080 seconds
Maximum Resident Size: 31200 KB
Page faults with physical i/o: 0
Memory usage for squid via mallinfo():
        total space in arena: 6038 KB
        Ordinary blocks: 5734 KB 9220 blks
        Small blocks: 197 KB 7040 blks
        Holding blocks: 0 KB 0 blks
        Free Small blocks: 14 KB
        Free Ordinary blocks: 289 KB
        Total in use: 5931 KB 98%
        Total free: 304 KB 5%

My disk are:
#df
Filesystem 1024-blocks Free %Used Iused %Iused Mounted on
/dev/lv01 3764224 0 100% 297341 32% /cache
/dev/lv02 4005888 113440 98% 308775 31% /cache2

My config for cache_dir in squid.conf is:
#cache_dir /usr/local/squid/cache 100 16 256
cache_dir /cache2 3500 8 256
cache_dir /cache 3500 8 256

Do I did something wrong? and why the disk will full and I was thought
it will maintain the side 3500mb itself?? why the disk full??
